To schedule an appointment, please provide us with the following information:
- Referral from your physician
- Your CNESST file number
- Accident date
- The name of your CNESST agent
- Your CNESST agent's phone and fax number
- CNESST Administration Form
Once we receive approval you will be contacted within 48 hours to schedule an appointment.
